An updated physical exam and immunizations are required for entry into 6th grade. The Certificate of Child Health Examination must be completed by a physician and dated within a year of the first day of school. The health history section must be completed and signed by a parent/guardian. Students entering 6th grade must show proof of receiving the following immunizations:
